ReachOut24 Home Care Services
We recognise the demand for home care services to respond to short-term care, long-term care and support needs, or episodically. Needs might arise due to recuperation from an operation or waiting for alternative living arrangements. Some older people using home care may be particularly vulnerable or have specialist needs, for example those with dementia; multiple long-term conditions; people who may be approaching the end of life; and people with mobility or communication difficulties.

We provide our clients with either 30 / 45 / 1 hour home calls or live-in care. ReachOut24 is here to help you live your life to the full and achieve better quality of care. Let us take the stress out of your hands. Our services include:
We assist clients with bathing, showering, washing, dressing, toileting, and bed changing.
Use of specialized equipment’s such as, hoists, slide sheets, commodes, walking aids. Our staff will be fully trained on the use of specialized equipment’s.
Assist clients getting up and getting ready for the day and help getting ready for bed.
In preparing meals with/ for clients and assisting with feeding if required.
Assist clients with tasks such as laundry, ironing, cleaning, tidying up and washing up.
Assist clients with collecting their pensions, organizing shopping list or accompany them for shopping trips
We assist you with collecting your medication from the pharmacy, arranging your medication, taking / reminding you to take your medication.
Accompany clients for walks, visiting friends or relatives, attending day centre, church or club, attending hospital appointments or important meetings or holidays.
